If you take a statin, worry about your cholesterol numbers, or have a family history of heart disease, this story is directly relevant to you. Eli Lilly published Phase 1 trial data in the New England Journal of Medicine showing that one infusion of VERVE-102 may permanently lower LDL, the most modifiable cardiovascular risk factor we have.
Here's how it works. Your liver uses LDL receptors to pull LDL cholesterol out of the bloodstream, but PCSK9 (proprotein convertase subtilisin/kexin type 9) continuously destroys those receptors, keeping LDL levels elevated.
VERVE-102 delivers a base editor, a molecule that swaps a single DNA letter inside the PCSK9 gene, turning it off permanently. The editor itself degrades after doing its job, but the corrected DNA gets copied to every new liver cell for life.
The Heart-2 Phase 1 trial results are striking. In the top dose group, PCSK9 dropped 88% on average and LDL fell 62%, all while 71% of participants were already on high-intensity statins and 43% were also on ezetimibe.
That stacking matters: this drug delivered a 62% additional LDL reduction on top of best-in-class existing therapy. A landmark 2006 NEJM study found that people born with natural PCSK9 loss-of-function variants had an 88% reduction in coronary heart disease risk over 15 years, and VERVE-102 is engineered to produce that same protection.
Now for the cautions worth knowing. This is Phase 1 data in 35 people, and there is no cardiovascular outcomes data yet: we don't know if the LDL reduction translates to fewer heart attacks, a question that won't be answered until Phase 3.
There is also an unresolved question around diabetes risk. Some studies of people with naturally occurring PCSK9 variants show a modest elevated risk of new-onset type 2 diabetes, though large outcome trials of existing PCSK9-blocking drugs and a UK Biobank study of 425,000 people found no association.
Pricing adds another layer of uncertainty. Eli Lilly's CEO has floated a model where patients receive the treatment and then pay an annual fee for as long as it keeps working, essentially a subscription attached to a one-time cure.
Cardiovascular disease remains the leading cause of death worldwide, and managing LDL with daily pills or biannual injections for decades is the current standard of care. VERVE-102 points toward a future where that disappears after a single clinic visit, with appropriately calibrated expectations about how much work remains.
Why Should You Care?
High LDL is still the most actionable cardiovascular risk factor you can address, and VERVE-102 represents the most promising permanent fix yet published in peer-reviewed science. The data is early, and the caveats are real, but the trajectory is one every longevity-minded person should have on their radar.
