If you already take a daily multivitamin, this study is the first large randomized trial to give you a concrete, measurable reason to keep doing it. And if you've been on the fence about whether supplements are worth it, this is a signal worth paying attention to.

The COSMOS trial randomized healthy adults into four groups: multivitamin plus cocoa extract, multivitamin plus placebo, cocoa extract plus placebo, or two placebos. Researchers pulled blood samples from 958 randomly selected participants and analyzed changes in five epigenetic clocks from the start of the trial through two years.

Epigenetic clocks work by reading DNA methylation patterns, which are chemical markers on your DNA that shift predictably as cells age. These clocks are among the most validated tools in biological age research.

The multivitamin group slowed in all five clocks compared to the placebo-only group. Two of those clocks are specifically tied to predicting mortality risk, and the slowing there was statistically significant.

The total effect translated to roughly four months less biological aging over the two-year study period. That may sound modest, but it is a measurable, reproducible deceleration produced by one of the most accessible and inexpensive interventions imaginable.

The finding that stands out most: participants who were biologically older than their chronological age at baseline showed the greatest benefit. If your cells have been aging faster than your years suggest, a daily multivitamin may be doing more work than you'd expect.

This is personally relevant because epigenetic aging rate is one of the most actionable longevity levers available today. Unlike genetic risk, which is fixed, the pace of your biological aging can respond to the choices you make.

A word of caution: this study measured a two-year window, and the researchers are planning follow-up work to determine whether the slowing persists after the trial ends. The study also cannot tell us which specific nutrients in the multivitamin are doing the most work.

Still, the COSMOS trial is the kind of large, randomized, placebo-controlled evidence the supplement world rarely produces. It moves multivitamins from the "probably harmless" column into the "measurable benefit" column.

Why Should You Care?
A daily multivitamin is one of the cheapest, most accessible interventions you can take, and this is the first large randomized trial to show it can measurably slow the biological aging process. If you're not already taking one, this study makes a compelling case to start.
