The Cocktail Report (sounds really smart around your friends):

  • On August 24 the FDA cleared Roche's Elecsys pTau217 plasma test, developed with Eli Lilly.

  • It is the first cleared test that uses a single marker to both rule in and rule out Alzheimer's plaque buildup, with the same cutoffs in family practice and at memory clinics.

  • It is cleared only for adults 55 and older who already have cognitive symptoms, not as a screening test for healthy people.

  • It runs on more than 4,500 Roche lab machines already installed across the country, so it is an ordinary blood draw.

  • Separately, a study of about 2,700 symptom-free adults found that very high levels of the same protein carried roughly a 78 percent chance of cognitive impairment within ten years.

If you are past 55 and have been quietly worried about your memory, the road to an answer just got much shorter. The test that used to require a specialty center can now be drawn at the same visit as your cholesterol panel.

The test measures phosphorylated tau 217, a chemically modified version of a brain protein called tau. It rises early in Alzheimer's disease and stays mostly normal in other brain conditions, which is what makes it useful.

Tau forms tangles inside brain cells. This particular version is valuable because its blood level tracks closely with amyloid, the sticky protein that clumps into plaques between brain cells and is the change doctors look for when confirming Alzheimer's.

Until now, confirming those plaques meant a PET scan, a specialized brain image made after injecting a tracer, or a spinal tap. Both are costly, invasive, and hard to get outside major medical centers.

That access gap has real consequences. Roughly 75 percent of dementia cases worldwide go undiagnosed, and diagnosis takes about three and a half years from the first symptom.

What is new here is the combination. Earlier cleared tests either measured two markers and calculated a ratio, or were meant mainly to rule Alzheimer's out.

This one does both jobs from one marker, and it reports results in three buckets: positive, intermediate, or negative. It also runs on lab equipment that thousands of hospitals already own, which is the difference between a scientific advance and an available one.

Worth noting how fast this happened. The first blood test of any kind for Alzheimer's cleared only in 2025, and this is already the fourth, the second to clear inside a single week.

Now the part the headlines blurred. Roche states plainly that this is not a stand-alone diagnosis, and that the test has not been shown to work for forecasting who will go on to develop dementia, or for tracking whether a treatment is helping.

That matters because a separate piece of research landed in July and got folded into the same news cycle. Researchers at Mass General Brigham and Harvard, publishing in the journal JAMA, followed about 2,700 cognitively healthy adults with an average age of 70.

Adults with very high levels of the protein had roughly a 78 percent chance of developing cognitive impairment over ten years, and about a one in three chance within five. Even slightly elevated levels carried a 15 percent risk at five years and 45 percent at ten.

Those are striking numbers, and they came from real long-term follow-up rather than a model alone. But that was a research study in symptom-free volunteers, which is not the population or the purpose the FDA cleared this test for.

If you take this request to your doctor, the more useful question is not whether you can get the test. It is whether a positive result would change what you do next.

For someone with symptoms, it often would, by shortening a long diagnostic odyssey and clarifying whether Alzheimer's explains what is happening. For someone with no symptoms, there is no approved treatment that acts on a positive result, and the researchers themselves said their numbers become more relevant only if prevention trials succeed.

One more practical wrinkle a neurologist raised: nobody yet knows how many patients will land in that intermediate gray zone, or what should be done with them. Kidney function, body weight, age, and genetics can all shift these protein levels too.

Why Should You Care?
A cheap blood test for Alzheimer's is now genuinely available at ordinary labs, which is a real change for anyone with memory complaints. Just know it is cleared to help explain symptoms you already have, not to forecast a disease you do not yet show.
